Bayambang, Dapat Alam Mo!

Anu-ano nga ba ang mga serbisyong ibinibigay ng LGU sa ating mga senior citizen?

Bayambang, dapat alam mo na -- sa pamamagitan ng MSWDO at Office of Senior Citizen Affairs -- kabilang sa ELDERLY WELFARE SERVICES ang mga sumusunod: 

- Maintenance and Updating of Senior Citizen Database ID        

- Issuance of Senior Citizen ID and Purchase Booklet

- Distribution of Purchase Booklet for Medicine and Grocery    

- Assistance to DSWD in Social Pension Pay-outs

Pagdating naman sa mga centenarian, dapat alam mo na ang isang residente ay itinuturing na centenarian kapag narating na ang edad na 100 sa araw ng pag-report sa opisina ng MSWDO.

Upang maclaim ang mandatory cash gift na P100,000 ng centenarian mula sa National Commission on Senior Citizens, kailangang magsumite ng mga sumusunod na requirements:

 Requirements

 1. Application Form

 2. Primary Identification

 • Photocopy of National ID (Philsys ID) or

• PSA Original or Certified True Copy of Live Birth Certificate

 3. Recent 2" by 2" ID Photo and Full Body Picture printed in A4 paper

 4. Endorsement Letter

 Endorsement letter from the Local Chief Executive where the applicant resides. For Filipinos abroad, an endorsement from the PE/Consulate, DFA, DMW, or CFO is acceptable.

 For Deceased Potential Beneficiaries

 a. Original death certificate.

b. Photocopy of a valid ID of the nearest relative.

c. Photocopy of proof of relationship (e.g., birth or marriage certificate).

d. If multiple relatives, a settlement must authorize one to receive the benefits and a release from liability form (Annex "B").

Note: Senior citizens need to be registered with the National Commission of Senior Citizens (NCSC).

Mayroon din silang natatanggap na counterpart cash gift mula sa LGU na nagkakahalaga ng P50,000.

Sa ilalim ng Expanded Centenarians Act, di lang mga senior citizen na edad 100 kundi pati na rin ang mga edad 80, 85, 90, at 95 ang maaaring makapag-avail ng cash grant mula sa National Commission of Senior Citizens.

Ang mga may nasabing edad ay makatatanggap naman ng P10,000 cash each.
